- Mature: 500/500 mg, every 6 hours or 1000/1000 mg, every 8 hours. The drug is given by infusion over 20-30 minutes. The maximum dose is 4,000/4,000 mg per day.
- Babies over 3 months old: 15/15 mg to 25/25 mg every 6 hours. The drug is given by infusion over 20-30 minutes. The maximum dose is 2,000/2,000 mg per day.
